ChenMed LLC is seeking a Primary Care Physician to provide comprehensive care for older adults, taking full accountability for patient outcomes and coordinating with hospitalists and care teams. You will lead a care team and work within a value-based, risk-sharing model.

The role requires MD/DO with license to practice in the state(s) and a focus on evidence-based, compassionate care for seniors, with opportunities for leadership and collaboration with medical directors and center teams.

Qualificações

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
• Current State Medical License for the state(s) you work in.
• Board certification preferred; Board eligibility required; ongoing maintenance of certification.

Responsabilidades

• Provide care for older adults with acute and chronic illnesses.
• Take full accountability for patient care and outcomes.
• Engage hospitalists when patients are admitted.
• Lead care teams and coordinate care across settings.
• Recruit patients and providers as needed in the market.
• Collaborate with APPs and maintain compliance with guidelines.
