Job summary

Katmai Government Services is seeking a qualified nurse case manager to coordinate healthcare services within a Military Treatment Facility setting in Colorado Springs. You will work with a multidisciplinary team to assess patient needs and develop comprehensive treatment plans that address medical and psychosocial factors while ensuring regulatory compliance.

The role requires an active nursing license, 3+ years medical/surgical experience with 1+ year in case management, and strong knowledge

Qualifications

• Bachelor of Science in Nursing from an NLN/AACN accredited program.
• Minimum of three years medical or surgical experience with at least one year in case management.
• Current, active nursing license in any US jurisdiction (compact or eNLC equivalence).
• Must maintain a valid State nursing license.
• Extensive knowledge of TRICARE, Medicare/Medicaid services and benefits.
• Experience with UM systems and care coordination.

Responsibilities

• Ensures compliance with standards of care and practice (JCAHO/AAAHC/CMSA).
• Collaborates with military and civilian healthcare providers using UM systems to identify high‑risk patients.
• Determines eligibility for healthcare coordination/case management services.
• Coordinates with providers to ensure appointment access and quality of care.
• Facilitates multidisciplinary team development of treatment plans and integrates education and prevention resources.
• Attends bi-weekly multidisciplinary discharge planning conferences and assists with referrals and directories.
• Maintains patient records and reports monthly summaries to the Director of Medical Management.
