Qualifications

• RN license active in any US state or territory.
• Minimum 2 years nurse case management experience.
• Case Manager certification (CCM, ACMA, or ANCC) or ability to obtain within 1 year of hire.
• Current Basic Life Support (BLS) certification.
• Ability to obtain and maintain a SECRET security clearance.

Responsibilities

• Conduct and document comprehensive health assessments.
• Develop time-sensitive, individualized care plans.
• Perform telephonic patient assessments per protocol.
• Monitor progress toward goals and adjust plans.
• Oversee discharge planning and continuity of care.
• Coordinate referrals to providers, facilities, and community resources.
• Collaborate with healthcare teams, vendors, and stakeholders.
