Core Responsibilities
Actively engage in all aspects of the CMP while ensuring compliance with CM standards of practice.
Provide specialized nursing insights throughout the CM process, which includes comprehensive assessment, strategic planning, implementation, coordination, and diligent monitoring. Recognize and implement local CM opportunities effectively.
Formulate and execute localized strategies utilizing diverse CM modalities, including inpatient, outpatient, onsite, and telephonic approaches.
Develop and implement supportive tools for case management operations, such as patient identification and assessment frameworks, clinical practice guidelines, algorithms, CM software, and databases that catalog community resources.
Integrate the realms of case management (CM) and utilization management (UM), ensuring synergy between nursing case management and social work case management principles.
Maintain collaborative relationships with pertinent community agencies and organizations to facilitate comprehensive care.
Accurately collect and document patient care data to support clinical decisions.
Design treatment plans that encompass preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions, ensuring continuity of care aligned with optimal wellness objectives.
Establish and monitor mechanisms that guarantee the effective implementation of patient treatment plans and follow-up processes after discharge in ambulatory and community healthcare environments.

Schedule referral appointments and coordinate specialty care as required.

Work Schedule
The established work hours are from 0730 to 1630, Monday through Friday, inclusive of a one-hour uncompensated meal break.
Minimum Qualifications
Education: Bachelor of Science in Nursing (BSN) from an ACEN or CCNE accredited institution.
Experience: A minimum of one year of case management experience, particularly with military Active Duty service members. Familiarity with Department of Defense (DoD) healthcare systems, Federal and State healthcare organizations, including DoD Tri-Service systems, TRICARE, and the Department of Veterans Affairs, is essential, alongside a basic understanding of the Recovery Care Coordinator program.
