Job summary

GLOM GLOBAL INC in Stockton, CA seeks a Mental Health Outreach Worker to coordinate services for clients transitioning from prison back into the community. You will provide support, arrange transportation to court, facilitate group sessions, and ensure basic needs are met.

The role emphasizes collaboration with county agencies, discharge planning, and confidential handling of sensitive information; prior experience in mental health or social services preferred but not required.

Qualifications

• College degree preferred but not required.
• Must have at least 45 college units with at least 30 units in psychology, social work, nursing or related social services field of study.

Responsibilities

• Complete initial and updated Needs and Service Plans (NSP's).
• Provide a summary of clients for all county meetings.
• Plan and facilitate weekly group sessions using the provided curriculum.
• Find and plan fun in-house activities for Clients.
• Transport to scheduled court appointments (as needed).
• Meet with each Client 1x1 for 30-60 min to discuss NSP goals, placement, or discharge.
• Write progress notes based on 1x1 and group interactions.
• Develop activity calendar monthly.
• Locate placements for clients.
• Preplacement interviews as needed, while completing the functional capability form and preplacement appraisals.
• Review client referrals.
• Meet with County case managers and conservators as needed.
• Accurately manage any appointments and communicate with the treatment team
• Report accordingly/crisis response team.
• Attend weekly team/clinical/1x1 meetings.
