Job summary

Transitional Services for New York, Inc. (TSINY) in Jamaica, NY, is seeking Service Coordinators for its PROS program to provide individual and group services to participants and manage a caseload with accuracy and compassion.

Qualifications

• Provide individual and group services to PROS participants.
• Manage an assigned caseload and complete paperwork per agency guidelines.
• Crisis intervention and on-call duties.
• Coordinate with community-based agencies for participants.
• Participate in supervision and staff training.
• Demonstrate ability to sit/stand and perform essential duties.

Responsibilities

• Provide caseload management for PROS participants.
• Complete IRP development, assessments, progress notes, and correspondence.
• Deliver individual and group services according to training.
• Maintain records and monitor service quality.
• Provide outreach to participants and community agencies.
• Respond to crises and participate in on-call coverage.
• Attend weekly supervision and ongoing program development.
