Tree of Life Counseling Center LLC is seeking a full-time LPN Intake Coordinator in Freehold, NJ. The role supports the PMHNP team, helping patients with scheduling, onboarding, and day-to-day customer service in a digital, paperless practice.

The position reports to the Nurse Practitioner Supervisor and involves coordinating appointments, discharge processes, referrals, and ensuring HIPAA-compliant records in a web-based EMR system.

Qualifications

• Licensure: Current, active, and unrestricted LPN or RN license in New Jersey.
• Experience in psychiatric or behavioral health setting preferred.
• Strong interpersonal and communication skills.
• Empathy and a compassionate demeanor.

Responsibilities

• Assist with patient discharge procedures.
• Monitor and report patient progress and changes to medical team.
• Reschedule appointments and manage referrals.
• Obtain prior authorizations for medications/treatments.
• Maintain accurate EMR records and confidentiality.
• Coordinate communication between patients, providers, and pharmacies.
