PermiaCare is seeking an Intake Coordinator to screen individuals seeking mental health services and link them with resources. This role supports crisis prevention, assessments, service planning, and coordination, often working independently under limited supervision.

The position requires a Bachelor's degree in a related field, at least 1 year of mental health experience, and the ability to obtain QMHP certification within 6 months.

Competenze

• Bachelor's degree in psychology, social work or related fields.
• At least 1 year experience in mental health field preferred.
• QMHP certification must be obtained within 6 months of hire.
• Valid Texas driver’s license and driving record acceptable.

Mansioni

• Assist clinician in screening potential mental health services patients.
• Assist clinician with coordinating intakes and completing consent forms for services.
• Make proper referrals to external services if patient does not qualify for services.
• Provide initial screening for all adults, children and adolescents seeking mental health services.
• Schedule follow up appointments in accordance with policies and Texas Administrative Code rules.
• Write service plans that incorporate the findings of the intake assessment and the individual choices of patient, while meeting the minimum requirements of current internal provider agreement.
• Ensure all intake paperwork is signed correctly and entered into the patient's chart in the EHR.
• Ensure progress notes clearly show progress or lack of progress in accordance with minimum requirements of current internal provider agreement.
• Meet unit performance measures or targets.
• Maintain assigned caseload of individuals with mental illness.
• Coordinate services to designated caseload.
• Enter accurate and appropriately documentation of services within timeframe required.
• Maintain confidentiality of sensitive records and treatment information in compliance with HIPAA, laws, rules and regulations, and established procedures.
• Maintain regular and reliable physical on-site attendance.
• Comply with Abuse, Neglect, and Exploitation policy and reporting requirements.
• Adhere to Code of Conduct and Standards of Behavior policy requirements.
• Establish and maintain effective work relationships with individuals served and their families, supervisors, co-workers and visitors.
• Communicate regularly with supervisor.
• Open and process mail/email in a timely manner.
• Answer phone, collect phone messages and respond to requests timely and accurately.
• Maintain safe and clean working environment by complying with procedures, rules and regulations.
• Perform all work functions using a trauma informed approach.
• Display professionalism when representing PermiaCare.
• Maintain compliance with legal requirements and company policies and procedures.
• Maintain valid and current driver's license, auto insurance, acceptable driving record and reliable transportation at all times.
