Qualifications

• Bachelor's degree in social work, psychology or related field.
• Five years' experience in a social service agency setting.
• Exceptional oral and written communication skills.
• Demonstrated ability to conduct and utilize screening and assessment tools.
• Demonstrated ability to establish relationships and linkages with community resources and advocate for quality service delivery.

Responsibilities

• Develops and maintains an appointment schedule for screenings and intake assessments.
• Conducts a screening, either by phone or in person, to determine applicability of agency's services to consumer’s needs.
• Verifies through the HFS Medi System, Managed Care Organization or other health insurance provider the client’s eligibility for mental health services and documents any limitations on the levels/types of care and/or pre-authorizations required.
• Gathers information from the consumer or referral source for eligible services, including psychiatric evaluation, mental health history, and other pertinent data.
• Presents and reviews referral information to the clinical team during Clinical Grand Rounds.
• Performs follow-up on requests for additional assessments and background data, respecting privacy and confidentiality.
• Contacts the consumer and/or family to schedule intake appointment if appropriate; informs about ineligibility and documents reasons.
