Job summary

PermiaCare is seeking Case Managers to work with individuals with mental illness and related conditions in Texas. You will manage a caseload, document accurately, and ensure timely service delivery as part of a dedicated team.

Qualifications include a Bachelor's degree in a relevant field and the ability to obtain QMHP certification within 6 months, plus a valid Texas driver’s license and background checks. This role offers full-time employment with comprehensive benefits.

Qualifications

• Bachelor's degree in psychology, social work or related fields required, with up to 30 credit hours in related areas.
• Minimum 1 year of experience in mental health or related field preferred.
• Must obtain QMHP certification within 6 months of hire.
• Must maintain a valid Texas driver's license and pass background checks.

Responsibilities

• Maintain an assigned caseload of individuals and provide required services.
• Keep accurate records and documentation within required timeframes.
• Meet unit performance measures and targets monthly to ensure services are delivered.
• Support both direct care staff and clients as part of a multidisciplinary team.
