Momentum in San Jose, CA is seeking a Case Manager to provide mental health rehabilitation and case management services to a diverse caseload. You will develop curricula, facilitate group and individual sessions, and assist clients and families with social integration into community life, housing, school, and employment opportunities.

Qualifications

• Master's degree in a mental health-related field and two years’ full-time experience in the mental health field.
• Bachelor's degree and four years’ full-time experience in the mental health field, or an associate degree with six years’ experience.

Responsibilities

• Provide mental health rehabilitation and case management services for a varying caseload, involving families to support daily living and community integration.
• Develop curriculum and facilitate groups and one-on-ones through rehabilitation counseling processes to build life and independent living skills.
• Assist clients and families with social integration into community life, housing, school, employment and other life-sustaining skills.
• Coordinate care with other programs; liaise with outside agencies and professionals; advocate for clients’ resources.

Education

Master's degree in a mental health-related field

Bachelor's degree (not necessarily mental health-related)
