This full-time PM shift role focuses on assessing clients, coordinating the interdisciplinary team, and implementing treatment plans with nursing staff to ensure continuity of care.

Qualifications

• Two years full-time experience in a mental health setting.
• Bachelor's degree in psychology, social work, or behavioral sciences or equivalent substituted for the two years of full-time work experience or its part-time equivalent.
• Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.

Responsibilities

• Assess clients to identify problem areas necessitating treatment.
• Coordinate interdisciplinary team meetings and discharge planning with the treatment team.
• Implement the individual treatment plan in collaboration with nursing staff to ensure continuity of care.
