Job summary

Qualifications

• Bachelor's Degree and 4 years of experience in a mental health setting.
• Up to 2 years of graduate professional education may substitute for experience.
• Experience with Medi-Cal Billing and Epic preferred.
• Knowledge of Supported Self-Management, Strength-Based and Recovery-Based services a plus.
• Effective assessment, advocacy, organizational and communication skills required.

Responsibilities

• Provide individual rehabilitation counseling, psychosocial case management, care coordination and crisis intervention.
• Assist residents with managing bills, purchases and financial matters.
• Offer resources to residents in a supportive, constructive, non-judgmental manner.
• Admit and orient new residents to the program and assess needs for medical/psychiatric evaluations.
• Develop and implement case management and money management plans; coordinate Plans of Care.
• Advocate for residents to secure entitlements, clothing, healthcare and services; ensure forms are completed.
• Lead groups and activities to promote stable living and community engagement.
• Meet monthly billing targets and maintain Epic documentation per Medi-Cal guidelines.
