WellLife Network is seeking a Case Manager to support ACT clients in delivering comprehensive case management services in New York. The role involves assisting with applications, entitlements, and CAIRS data entry, while coordinating care with clinicians and the ACT team.

The ideal candidate has at least 2 years of experience with mental health populations and demonstrates strong documentation, communication, and teamwork skills to help clients achieve their service goals.

Qualifikationen

• High school diploma or GED required; Bachelor's in human services preferred.
• Minimum 2 years' experience working with individuals with mental illness.
• Experience with behavioral health populations preferred.
• Proficient computer skills; able to enter documentation into EMR/EHR.

Aufgaben

• Provide supportive services to assist ACT recipients with applications and entitlements.
• Complete SPA and SPOA applications with clinician support.
• Input data into CAIRS at admission, bi-annually, and discharge.
• Document progress notes in EHR within 48 hours of contacts.
• Coordinate case management services and transportation as needed.
• Attend team meetings and participate in supervision.
