Job summary

WellLife Network in Elmont, NY is seeking a skilled Case Manager to provide support and service linkages to residents of Abbey Manor CR/SRO, coordinating services and ensuring quality care. The role involves direct care, documentation, crisis intervention, and travel between sites with a flexible schedule.

You will participate in meetings/trainings, maintain consumer files, and help plan recreational activities, contributing to the safety and well-being of individuals served by the program.

Qualifications

• Bachelor’s Degree preferred with 1 year working with homeless or mentally ill individuals.
• Associates Degree with 2+ years working with special populations.
• High School Diploma with 3 years experience with special populations.

Responsibilities

• Provide direct care and develop goals/skills in daily living activities.
• Coordinate services, document progress, and maintain consumer files monthly.
• Assist with crisis intervention, incident reporting, and safety planning.
• Attend case management meetings and participate in trainings at various sites.
• Coordinate transportation, scheduling, and recreational activities as needed.
