Job summary

Well Care Services is seeking a full-time Mental Health Case Manager to provide field-based services to adults with serious mental illness in the Las Vegas area. You will assess clients' strengths and needs and develop individualized service plans to help them reach their recovery goals.

Responsibilities include completing service assessments, linking consumers with community resources, coordinating services, and monitoring goal attainment while collaborating with other professionals.

Qualifications

• Minimum of two years of experience working with adults with serious mental illness or equivalent training/experience.
• Experience with interview techniques, service planning, and consultation.
• Familiarity with electronic medical records and ability to type.
• Good driving record; must not have six or more negative points in past three years or major violation in past four years.

Well Care Services is a Behavioral Health Clinic looking for Case Managers to join our team. We are hiring a team of case managers that will be working hands on with patients out on the field.

Requirements:

Minimum of two years of experience working with adults with serious mental illness; or an equivalent combination of training and experience. Knowledge of serious mental illness, substance abuse, psychotropic drugs, entitlement programs and recovery oriented services required. Interviewing, service planning, consultation, public relations, and experience with electronic medical record. Computer skills and the ability to type required. Good driving Must not reflect a total accumulation of six or more negative points within the past three years or a major violation within the past four years.

Duties:

Well Care Services is seeking a full time qualified Mental Health Case Manager to provide services to clients with mental illness. Functions include assessing clients' strengths and needs; developing individualized service plans; completing services assessments; linking consumers with other community resources; coordinating services including health care and monitoring sufficiency of services and goal attainment; collaboration with other professionals.
