Job summary

WellLife Network in New York's Elmont area seeks a Case Manager to provide support and service linkages to Abbey Manor residents, coordinating services and ensuring quality care in residences, day treatments and habilitation facilities. The role involves direct care, goal planning, documentation, crisis intervention, and transportation coordination.

Qualifications include a Bachelor’s degree or equivalent and experience with homeless or mentally ill populations.

Qualifications

• Bachelor’s Degree preferred with a minimum of 1 year working with homeless, mentally ill chemical abusers or; An Associates Degree and 2 or more years working with special populations or; A High School Diploma and 3 years experience with special populations.

Responsibilities

• Provide direct care services to consumer, including planning and implementation of goals and training, which will develop and enhance activities of daily living skills.
• Coordinate evacuation and ensure consumers’ safety in emergency situations.
• Maintain confidentiality and complete required documentation including progress notes and service plans.
• Participate in meetings/trainings at various sites and support recreational/social activities for consumers.
• Transport to various sites as needed and communicate with staff for coordinated care.
