Job summary

WellLife Network is seeking a Case Manager to provide support and service linkages to residents of Abbey Manor CR/SRO in Bronx, NY. The role involves coordinating services, documenting progress, and ensuring quality care across residences, day treatments, and habilitation facilities.

The position requires a background in social services, the ability to transport to multiple sites, and a commitment to compassionate, person-centered care.

Qualifications

• Bachelor’s Degree preferred with minimum 1 year working with homeless, mentally ill chemical abusers; Associates Degree with 2+ years; High School Diploma with 3 years experience with special populations.
• Experience with homeless populations and/or special populations is preferred.
• Ability to provide direct care services, documentation, and coordination of services.

Responsibilities

• Provide direct care services to consumers, including planning and implementing goals and training to develop daily living skills.
• Coordinate evacuation and ensure consumer safety in emergencies.
• Prepare and maintain written documentation: assessments, medication updates, progress notes, and quarterly service plans.
• Assist consumers in planning recreational activities and manage consumer files monthly.
• Attend meetings/trainings at various sites and transport self to sites.
• Communicate effectively with staff and supervisors, and participate in crisis intervention as needed.
