Job summary

Mental Health Resource Center is seeking a Care Coordinator for the Suicide Prevention Team. This role emphasizes rapid follow-up, care coordination, and transition planning for individuals after discharge from emergency or inpatient settings.

Qualifications

• Bachelor’s degree in Social Work or a related Human Services field required.
• Minimum of two years of experience in human services or behavioral health preferred.
• Proficiency in MHRC EHR System within three months of employment; proficiency in Microsoft Office and email required.

Responsibilities

• Perform initial and ongoing assessments including suicide risk, medical history, and living situation.
• Engage crisis stabilization units, hospitals, and community partners to coordinate care.
• Develop individualized care and safety plans with individuals, families, and providers.
• Provide safety planning and follow-up to ensure continuity of care.
