Job summary

CN Guidance & Counseling Services, Inc. is seeking a Care Coordinator to join an interdisciplinary team providing care coordination for adults with severe mental illness and co-morbid conditions.

You will advocate for clients, connect with community providers, and support wellness self-management to reduce ER visits and hospitalizations. Requirements include a Bachelor's or Master's in a relevant field, experience with mental health services, and strong documentation and communication skills.

Qualifications

• Bachelor's or Master's degree in Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, or related field preferred.
• Experience with individuals with severe mental illness and co-occurring disorders; link clients to services and advocate.
• Verbal and written communication skills; documentation per program standards.
• Knowledge of Medicaid, Social Security, and entitlements preferred.
• Ability to travel to multiple locations; valid NY tri-state driver's license if required.

Responsibilities

• Conduct initial and ongoing assessments to document client strengths, needs, goals and resources.
• Develop and update client-centered care plans in collaboration with the team.
• Maintain effective communications with clients, physicians, providers, family and staff.
• Coordinate care planning across providers to ensure goal-directed, collaborative care.
• Provide outreach, engagement and service planning in the field and via telephone.
