Job summary

Lighthouse Behavioral Wellness Centers is seeking a Care Coordinator to empower and help individuals heal from mental illness, trauma or addiction. You will advocate, link consumers to services after crises or hospitalization, and work with an integrated team across inpatient and outpatient settings.

Under supervision, you will assess needs, develop care plans, coordinate with internal and external providers, and monitor progress while providing crisis stabilization and referrals to housing,

Qualifications

• Knowledge of social work/psychology principles and current health resources.
• Understanding Eight Dimensions of Wellness and personal wellness.
• Ability to coordinate care across inpatient/outpatient settings and with providers.

Responsibilities

• Provide individualized case management, rehabilitation and wellness services in various settings.
• Provide crisis management and stabilization as assigned.
• Monitor consumer needs and progress, using community resources.
