Job summary

Cherokee Park Rehabilitation is seeking a Memory Care Coordinator. We are looking for a skilled Registered Nurse (RN) with a compassionate spirit to organize, plan, and facilitate activity programs for Memory Care residents in a 20-bed all-female unit.

The role emphasizes program development, human resources support, and the ability to lead engaging activities while collaborating with a multi-disciplinary team. There is a strong focus on resident wellness and meaningful engagement.

Qualifications

• Active RN license is required.
• Experience as a program coordinator or similar role.
• Knowledge of program management and development procedures.
• Ability to work with diverse, multi-disciplinary teams.
• Excellent time-management and organizational skills.
• Strong verbal and written communication abilities.
• Detail-oriented and efficient in execution.

Responsibilities

• Lead and coordinate memory care activity programs to enhance residents' well-being.
• Offer diversified recreations with supervision and ensure meaningful engagement.
• Implement facility policies and practices.
• Motivate residents to participate in programs.
• Foster positive relationships with team and external partners.
• Schedule and organize meetings and events.
• Inform residents of upcoming activities and maintain event calendars.
• Maintain records of activities, participation, and compliance with state regulations.
• Support program development and growth.
• Perform other duties as needed.
