Job summary

Recover-Care is seeking a Memory Care Coordinator to lead dementia-focused care planning and program development for residents with Alzheimer’s and related cognitive impairments. This role requires clinical oversight, staff education, and collaboration with nursing, therapy, activities, physicians, residents, and families to promote dignity and quality of life.

Qualifications

• Graduate of an approved school for practical or vocational nursing.
• Must be currently licensed by the State Board of Nursing.
• Minimum 1–3 years of experience in long-term care, skilled nursing, memory care, or related healthcare setting.
• Experience with residents diagnosed with Alzheimer’s disease and related dementias preferred.
• Current CPR certification.
• Dementia Care Certification preferred or willingness to obtain.
• Able to read, write, speak and understand English to ensure patient safety.

Responsibilities

• Assess residents’ cognitive, physical, psychosocial, and behavioral needs.
• Participate in development, implementation, and review of individualized care plans.
• Monitor resident progress and revise care plans as needed.
• Assist in identifying behavioral triggers and implementing non-pharmacological interventions.
• Attend and participate in resident care conferences and family meetings.
• Collaborate with the interdisciplinary team to ensure comprehensive care.
• Provide education and support to families regarding memory care.
