Job summary

Cherokee Park Rehabilitation is looking for a compassionate Registered Nurse to lead memory care activity programs for a 20-bed all-female unit. This RN will organize, plan, and facilitate wellness activities and collaborate with the care team.

The role emphasizes program development, documentation, and building relations with residents and external partners while upholding state requirements and facility policies.

Qualifications

  • Must hold an active RN license.
  • Experience as program coordinator or relevant role.
  • Strong knowledge of program management and development.
  • Ability to work with diverse, multi-disciplinary teams.
  • Excellent time management and organizational skills.
  • Strong verbal and written communication skills.
  • Detail-oriented and efficient.

Responsibilities

  • Lead programs that engage residents and enhance well-being.
  • Provide diversified recreational activities with supervision.
  • Ensure implementation of facility policies.
  • Motivate residents to participate and maintain activity records.
  • Foster positive relations with staff and external partners.
  • Schedule meetings and events.
  • Inform residents of upcoming activities and maintain event calendars.
  • Maintain records of activities and participation as required by state law.
  • Support program growth and development.
  • Perform other duties as needed.

Job description

Cherokee Park Rehabilitation is seeking a Memory Care Coordinator!

We are looking for a skilled Registered Nurse (RN) for this position. You must be a people person with a compassionate spirit and an enthusiasm for working with Memory Care residents. This individual is responsible for the organization, planning, and facilitation of activity programs designed to promote the overall wellness of our Memory Care resident population. It is a 20-bed, all-female unit. If you have skills in program development and human resources support, we’d like to meet you!

Responsibilities
  • Leads assigned programs that are of interest and meaning to our residents in order to enhance their lives and exceed their active aging lifestyle expectations.
  • Offers adequate and diversified recreational activities to residents with sufficient supervision for each activity.
  • Ensure implementation of policies and practices.
  • Engages and motivates residents resulting in program participation.
  • Help build positive relations within the team and external parties.
  • Schedule and organize meetings/events.
  • Informs residents of upcoming activities and maintains a current schedule of events.
  • Maintains records of all activities, resident participation levels and acceptance of each activity by residents as required by state law.
  • Support growth and program development.
  • May perform other duties as needed and/or assigned.
