River Grove Health and Rehab in Tennessee is seeking a compassionate Memory Coordinator to lead engaging, person-centered activities for residents in our Memory Care Unit. The role focuses on cognitive stimulation, emotional well-being, and social connection.

You will collaborate with nursing and social services, adapt activities to memory loss, and help create a warm, home-like environment where residents feel valued and empowered.

Qualifications

• Experience running an activities department in long-term care.
• Must have a background as an Activities Director, Certified Occupational Therapy Assistant, or in social work.
• Excellent communication, creativity, and interpersonal skills.
• Compassionate, patient, and adaptable when working with individuals experiencing memory loss.
• Ability to work collaboratively as part of an interdisciplinary care team.

Responsibilities

• Plan and coordinate a structured activity program designed specifically for dementia residents, incorporating daily routines, therapeutic recreation, and individualized engagement opportunities.
• Facilitate group and one-on-one activities such as music, art, sensory experiences, exercise, reminiscing sessions, and life enrichment events that align with residents’ interests and abilities.
• Adapt activities to meet the changing physical and cognitive abilities of each resident, ensuring all participants can engage at their comfort level.
• Provide emotional and social support by building trusting relationships with residents, offering companionship, and encouraging participation and self-expression.
• Collaborate with nursing and social services teams to support residents’ care plans, monitor changes in behavior or mood, and communicate updates effectively.
• Maintain accurate documentation of resident participation, activity attendance, and behavioral observations as required.
• Assist in creating a positive, home-like atmosphere that supports the emotional well-being and dignity of all residents in the Affinity Care Unit.
• Contribute to special events and family involvement activities, promoting community engagement within the unit.
