Qualifications

• Experience or training in direct care for older adults living with dementia preferred.
• Ability to follow person-centered care plans and state regulations for medications as applicable.

Responsibilities

• Assist residents with bathing, dressing, grooming, and mobility per care plans.
• Serve meals, monitor nutrition and hydration, and document changes in health condition.
• Maintain accurate daily records including care provided and observations.
• Support safety and independence, participate in team meetings and care plan updates.
