Job summary

Qualifications

• LPN, CNA, or HCA license.
• Previous experience in assisted living.
• High school diploma or equivalent.
• Meets all mandatory health and training requirements.
• Maintains CPR and First Aid certifications.
• Demonstrates basic computer skills.
• Flexible schedule including weekends/holidays.
• Strong communication with residents, families and staff.
• Able to work independently and read/interpret medical information in English.

Responsibilities

• Supervises and evaluates resident care in memory care and assisted living.
• Schedules and trains care staff and ensures regulatory compliance.
• Coordinating patient care plans with providers to meet needs.
• Coordinate follow-up appointments and referrals to specialists.
• Improve quality of patient care and create efficient workflows.
• Attend staff meetings and training to stay updated on clinic initiatives.
• Meet with residents and families to address needs and resolve issues.
• Maintain up-to-date certifications and adhere to health requirements.
