Job summary

Rich Square Manor and Villa is seeking a compassionate Special Care Coordinator to lead Memory Care services, develop individualized care plans, and train staff to promote dignity and engagement for residents with cognitive impairments.

You will collaborate with the Executive Director, nursing team, families, and care staff to support a safe, nurturing environment and continuous quality improvements in memory care.

Qualifications

• Bachelor’s/Associate degree in nursing, healthcare admin, or related field.
• Active LPN or RN license preferred (state dependent).
• 2+ years in senior living, memory care, or healthcare setting.

Responsibilities

• Lead memory care care planning and program development.
• Supervise and train care staff in dementia care principles.
• Engage residents and families; coordinate with executive leadership.
