Job summary

Kingsbridge House is seeking a Special Care Coordinator to lead memory care services, develop person-centered plans, and coordinate with care teams to support residents with cognitive impairments.

The role emphasizes staff training, family engagement, and ensuring adherence to state regulations and quality standards to maintain a safe, nurturing environment tailored to residents’ needs.

Qualifications

• Associate’s or Bachelor’s degree in Nursing, Healthcare Administration, or related field preferred.
• Active LPN or RN license preferred (depending on state requirements).
• Minimum of 2 years of experience in a senior living, memory care, or healthcare setting.
• Previous supervisory or leadership experience in a healthcare environment is highly desirable.
• Strong leadership, communication, and interpersonal skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Knowledge of state regulations and compliance standards for assisted living and memory care.
• Proficiency in electronic health records (EHR) systems and basic computer skills.

Responsibilities

• Care Plan Development & Management: Develop, implement, and evaluate memory care programs.
• Staff Supervision & Training: Lead, coach, and train care staff in dementia care principles.
• Resident & Family Engagement: Be primary contact for residents and families; coordinate meetings and updates.
• Regulatory Compliance & Quality Assurance: Ensure compliance with state regs and company policies; conduct audits.
• Collaboration: Work with nursing, physicians, therapists, and leadership to coordinate services.
