Job summary

Qualifications

• Experience with memory impairment and/or dementia care.
• Supervisory experience in healthcare, social services, or adult day programs.
• Strong organizational, communication, and leadership abilities.

Responsibilities

• Oversee all aspects of the Memory Impaired program, including admissions and scheduling.
• Supervise, train, and support staff, interns, and volunteers.
• Complete and maintain social assessments, care plans, and required documentation.
• Lead care planning meetings and share plans with caregivers and partners.
• Coordinate caregiver support groups and collaborate with external agencies.
• Partner with transportation staff to ensure safe and efficient services.
• Respond to registrant needs and emergencies as required.
• Maintain ongoing communication with program leadership.
