Qualifications

• Bachelor's or Master's degree in nursing, healthcare administration, social work, public health, gerontology, or related field (or equivalent).
• 5+ years in dementia care, care coordination, value-based care, or population health with supervisory experience.
• Proven ability to run day-to-day healthcare program operations and support a care-coordination team.
• Active clinical license (RN or LPN) preferred.
• Strong knowledge of dementia care, caregiver support, and interdisciplinary delivery.
• Experience with EMRs and quality dashboards.

Responsibilities

• Oversee day-to-day operations of the GUIDE Program per CMS guidelines.
• Personally manage a portion of the GUIDE patient panel and care plans.
• Coach and evaluate Memory Care Coordinators and handle escalations.
• Coordinate respite services and partnerships with in-home providers.
• Develop dashboards and KPIs to monitor program outcomes.
• Identify and enroll eligible beneficiaries; manage referrals.
• Communicate with CMS and community partners to ensure access to services.
• Maintain compliance with CMS GUIDE model and data reporting.
