Job summary

Episcopal SeniorLife Communities seeks a Director of Community Dementia Services to lead operationally across GUIDE care navigation, caregiver support, respite coordination, and dementia-friendly programming.

You will oversee Medicare GUIDE operations, caregiver education and partnerships, while driving program growth and sustainability with cross-functional teams and external partners.

Qualifications

• Bachelor’s degree in social work, public health, health administration, community health, gerontology, nursing, human services, or related field required.
• Master’s degree preferred.
• Experience with Alzheimer’s/dementia care or related programs is preferred.
• Proven leadership and staff supervision experience across a multidisciplinary team.
• Strong communication, grant reporting and presentation skills.
• Knowledge of privacy, compliance, and billing readiness in healthcare-adjacent settings.

Responsibilities

• Provide operational leadership for the Community Dementia Services portfolio including GUIDE care navigation, caregiver support, and respite coordination.
• Translate priorities into workflows, staffing plans, and measurable goals to ensure high-quality service delivery.
• Monitor program quality, participant and caregiver experience, referrals and enrollment activity.
• Oversee budgets, fiscal stewardship and grant/payer reporting.
• Identify opportunities for improvement and coordinate program growth and sustainability.
• Coordinate dementia services as an integrated service line with GUIDE model delivery and related programs.
