Job summary

Symphony Pointe in Elkhorn, NE seeks an experienced Licensed Practical Nurse to act as Memory Care Manager, leading a team and delivering compassionate care for residents with memory loss.

You will oversee care plans, regulatory compliance, and staff development while partnering with families and providers to ensure personalized memory care under EPIC ACTS values.

Qualifications

• Licensed Practical Nurse (LPN) license valid in Nebraska is required.
• Minimum 2+ years of leadership or supervisory experience.
• Experience with seniors, memory care preferred.
• Strong clinical judgment and communication skills.

Are you an LPN ready to bring your clinical experience and leadership skills to memory care?

At Symphony Pointe, an Agemark Senior Living community, we’re looking for a Licensed Practical Nurse (LPN) to serve as our Memory Care Manager, leading the team and helping create exceptional experiences for residents living with memory loss. What You’ll Do

• Lead, coach, and support the Memory Care team while building a positive, accountable culture
• Oversee resident care plans, documentation, safety practices, and regulatory compliance
• Use your clinical knowledge to support high-quality, individualized resident care
• Partner with families, healthcare providers, and team members throughout each resident’s memory care journey
• Bring personalized memory care to life through the Best Friends™ Approach
• Support engaging programs and experiences that create connection and purpose
• Manage departmental operations, budgets, and staffing
• Help keep the Memory Care neighborhood welcoming, organized, and running smoothly

What You’ll Bring

• Current Licensed Practical Nurse (LPN) license valid in Nebraska required
• 2+ years of leadership or supervisory experience
• Experience working with seniors; memory care experience strongly preferred
• Strong clinical judgment, communication, and problem-solving skills
• Knowledge of regulatory requirements, OSHA standards, and safety practices
• A compassionate, approachable, and accountable leadership style
• Comfort with computers and technology
• Flexibility to work evenings and weekends and step in when needed
