CountryHouse Lincoln II is seeking a Health & Wellness Director to lead the wellness team and deliver person-centered care for residents in Lincoln, NE. If you value compassion, teamwork, and excellence in elder care, join a supportive Agemark community recognized for workplace excellence.

You’ll manage clinical plans, supervise staff, and collaborate with families and healthcare partners to ensure safe, high-quality services every day.

Qualifications

• Active Nebraska nursing license.
• At least 1 year of senior care experience.
• At least 2 years of leadership or management experience.
• Strong understanding of medications, dosages, and potential side effects.
• Comfortable using software and electronic systems.
• Availability for evenings, weekends, and Manager on Duty responsibilities.

Responsibilities

• Lead and develop wellness team, coaching and supporting staff.
• Be present and involved with residents and staff, providing hand-on support.
• Create personalized care plans based on residents' needs, assessments, and physician orders.
• Build strong relationships with residents, families, and healthcare providers to ensure clear communication and coordinated care.
• Oversee resident care to ensure safe, compassionate, person-centered services.
• Oversee medication management practices and promote accuracy and safety.
• Assess new residents to determine best fit for the community.
• Ensure compliance with documentation, state regulations and Agemark standards.
• Support safety and wellness by identifying risks and responding to emergencies.

What You'll Do

• Lead & Develop Your Team: Coach, support, and grow wellness team members while creating a positive, collaborative environment.

• Support Safety & Wellness: Identify changes or risks, respond to emergencies, and implement proactive plans to support resident well-being.
• Make a Positive Impact: Help create a community where residents feel known, respected, supported, and at home.

What We're Looking For

• RN or LPN: Active Nebraska nursing license.
• Senior Care Experience: At least 1 year of experience working with seniors.
• Leadership Experience: At least 2 years of experience leading or managing a team.
• Medication Knowledge: Strong understanding of medications, dosages, and potential side effects.
• Strong Leadership Skills: A compassionate, dependable leader with excellent communication and problem-solving abilities.
• Tech Savvy: Comfortable using software and electronic systems.
• Flexibility: Availability to support the community as needed, including occasional evenings, weekends, and Manager on Duty responsibilities.
