River Grove Health and Rehabilitation is seeking an LPN for our Affinity for Her memory care unit. You will provide compassionate bedside care, monitor vital signs, administer medications, and collaborate with RNs and physicians to support residents in a safe, engaging environment.

Qualifications

• Current LPN license in the state.
• At least one year of LPN experience in a long-term care setting preferred.

Responsibilities

• Provide bedside care to residents.
• Record vital signs (BP, pulse, respiration).
• Administer oral medications and injections.
• Test blood sugar levels.
• Assist with personal hygiene.
• Relay information between patients, RNs and physicians.
• Maintain patient medical records and documentation.
• Ensure confidentiality of patient information.
