AuthentiCare Health Centers is seeking an LPN to join our caring family in our skilled nursing facilities. You will provide resident-centered care, administer medications, and collaborate with physicians to deliver high-quality memory care and sub-acute services.

Qualifications

• Must be a registered or licensed nurse in the state.
• Experience in long-term care or skilled nursing preferred.
• Ability to administer medications and care plans per physician orders.

Responsabilités

• Assess resident health problems and develop nursing care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor resident progress and respond to changes in condition.
• Collaborate with multidisciplinary teams to provide comprehensive care.
• Educate residents and families regarding health conditions and discharge plans.
• Maintain accurate resident records in compliance with state and federal regulations (HIPAA).
• Participate in case management to ensure continuity of care.
• Support in memory care, sub-acute, and related services.
