Job summary

The Oars Senior Living in Citrus Heights, CA is seeking a Care Manager to provide quality care for residents with health ailments and cognitive impairment. You will report to the Assisted Living Director or Memory Care Director and help create a respectful, supportive environment.

You will build compassionate relationships with residents and families, assist with daily living activities, participate in interdisciplinary care, and maintain a clean, positive community.

Qualifications

• Must be at least 18 years old.
• High School Diploma or equivalent required.
• CNA certification helpful, but not required.
• Experience with aging or dementia care preferred.

Responsibilities

• Develop a naturally inspired relationship with residents and families.
• Ensure care delivery is dignified and of quality standards.
• Assist residents with activities of daily living according to their plan of care.
• Active involvement in interdisciplinary operations for Memory Care and Assisted Living.
• Daily light housekeeping and maintaining a positive environment.
• Report changes in resident condition to Directors promptly.
• Comply with community rules, policies and regulatory standards.
