Job summary

Saber Healthcare Group is seeking a Memory Care Neighborhood Coordinator Assistant in our skilled nursing facility to lead dementia-capable care and model best practices. You will provide direct resident engagement, behavioral support, and mentor staff while overseeing cognitive, sensory, and life enrichment programming to ensure safe, person-centered care for residents living with dementia.

Qualified applicants hold a valid CNA/STNA certification in the operating state, with Dementia

Qualifications

Valid Certified Nurse Aide (CNA/STNA) certification is required in the state in which you operate.
Dementia Practitioner Certification preferred or ability to obtain within six (6) months of hire.
Minimum two (2) years’ experience in long-term care, memory care, or dementia-focused programming.

Responsibilities

Lead dementia-capable care within the Memory Care Neighborhood.
Provide direct resident engagement, behavioral support, and staff mentoring.
Oversee structured cognitive, sensory, and life enrichment programming.
Ensure person-centered, safe and therapeutic care for residents living with dementia.
