The Memory Care Neighborhood Coordinator Assistant is responsible for leading and modeling dementia-capable care within the Memory Care Neighborhood following plans put in place by the Memory Care Coordinator (MCC). This role provides direct resident engagement, behavioral support, staff mentoring, and oversight of structured cognitive, sensory, and life enrichment programming. The Coordinator Assistant will help ensure person-centered, safe, and therapeutic care practices for residents living with dementia.
Valid Certified Nurse Aide (CNA/STNA) certification is required in the state in which you operate
Dementia Practitioner Certification preferred or ability to obtain within six (6) months of hire
Minimum two (2) years' experience in long-term care, memory care, or dementia-focused programming.
