Job summary

Gardant-Management-Solution seeks a skilled nursing leader to design and oversee the Community Memory Care program in Illinois. The role requires direct leadership of the Memory Care clinical team, development of individualized care plans, and active engagement with residents and families to promote wellness and independence.

Responsibilities include budget oversight, staff training, and implementation of health promotion and dementia education services, while maintaining a safe, welcoming

Qualifications

• Must have RN or LPN licensure; Dementia Care Certification preferred.
• Minimum of three (3) years of dementia care experience.
• Strong leadership, mentoring and motivational skills.
• Excellent written and verbal communication abilities.

Responsibilities

• Designs and facilitates the Community Memory Care program.
• Oversees budget and implements cost-effective Memory Care program; manages inventory and resources.
• Creates a welcoming, safe environment promoting resident wellness and independence.
• Participates in move-in assessments and develops individualized care plans.
• Trains, schedules, and leads Memory Care clinical team; provides hands-on support and training.
• Delivers health promotion and dementia care education to residents and families.
• Engages with residents and families with compassion to create lasting experiences.
• Contributes to quality assurance and performance improvement initiatives.
• Coordinates infection control programs and ensures regulatory compliance.
