Job summary

Frank Residences in San Francisco's Excelsior neighborhood seeks a Care Coordinator to lead day-to-day care coordination for Memory Care residents. You will report to the Director of Memory Care and support the Health & Wellness team by modeling dementia care best practices and training staff.

You will also assist with appointment coordination, documentation compliance, and onboarding for non-employee staff, while promoting resident dignity, safety, and family-centered care.

Qualifications

• High school diploma or equivalent required.
• One year+ experience in residential care or dementia programs preferred.
• Strong interpersonal, customer service, and problem-solving skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Excellent verbal and written communication skills.
• Ability to work collaboratively in a team.
• Proficient in Microsoft Office and electronic health record systems.
• English reading/writing proficiency.
• Reliable attendance and punctuality.

Responsibilities

• Lead day-to-day care coordination for Memory Care residents.
• Support Health & Wellness team with dementia care best practices.
• Coordinate and document appointments and provider follow-up.
• Ensure compliant documentation and HIPAA adherence.
• Assist with staffing coordination and schedule adjustments.
• Provide onboarding and training for memory care staff.
• Escort residents to appointments and activities.
• Observe changes in condition and report to leadership.
