Job summary

San Francisco Campus for Jewish Living seeks a dedicated Care Coordinator to lead day-to-day care coordination for Memory Care residents at Frank Residences. You will support the Health & Wellness team by modeling dementia care best practices, coordinating appointments, and ensuring compliant documentation.

You will work with residents, families, and staff to address care needs, provide onboarding or training for memory care staff, and help maintain high safety and dignity standards in a

Qualifications

• High school diploma or equivalent required.
• Experience providing residential care and/or leading programs for older adults preferred.
• Strong interpersonal and problem-solving skills.
• Ability to manage multiple priorities in a fast-paced setting.
• Excellent written and verbal communication in English.
• Ability to work collaboratively within a team and with families.

Responsibilities

• Lead day-to-day care coordination for Memory Care residents under supervision.
• Support the Health & Wellness team by modeling best practices and providing staff coaching.
• Coordinate appointments, follow-up with providers, and maintain compliant documentation.
• Assist with staffing coordination and coverage to ensure resident safety.
• Document resident services, ADLs, and changes in condition accurately.
• Communicate with residents, families, and staff regarding care needs and concerns.
