American Senior Communities LLC is seeking a Memory Care Support Specialist for Meadow Lakes. This role develops and coordinates person-centered programming for residents with Alzheimer's and other dementias, partnering with interdisciplinary teams to promote meaningful engagement and educate staff.

The position emphasizes compassionate, resident-centered care, collaboration with nursing, therapy, and social services, and adherence to Memory Care Program standards.

Qualifications

• Bachelor's degree preferred in social work, therapeutic recreation, psychology, human services, healthcare administration, or a related field.
• Experience with residents with Alzheimer's/dementia in long-term care required.
• Certification in dementia care preferred.

Responsibilities

• Develops, coordinates and supports person-centered programming for residents with Alzheimer's/dementia.
• Partners with interdisciplinary teams to promote engagement and educate staff.
• Upholds the philosophies and standards of the Memory Care Program.
• Collaborates with nursing, therapy, social services and activities to promote resident care.
• Promotes resident quality of life and meaningful engagement.
