Job summary

American Senior Communities is seeking a Memory Care Program Coordinator at Allisonville Meadows to lead person-centered dementia programming and collaborate with nursing, therapy, social services, and leadership teams. You will support residents with Alzheimer's and related conditions while promoting compassionate, high-quality care.

You will educate staff, coordinate activities, and uphold the CARE values of Compassion, Accountability, Relationships, and Excellence across the memory care team

Qualifications

• Experience working with residents living with Alzheimer's disease or other forms of dementia in long-term care is required.
• Bachelor's degree in Social Work, Therapeutic Recreation, Psychology, Human Services, Healthcare Administration, or related field preferred.
• Professional certification in dementia care or Alzheimer's care is preferred.
• Demonstrates C.A.R.E. values: Compassion, Accountability, Relationships, Excellence.

Responsibilities

• Develops, coordinates, and supports person-centered programming for residents with dementia.
• Partners with interdisciplinary teams to promote meaningful engagement and quality of life.
• Educates staff and upholds Memory Care Program standards.
