Job summary

PACS in Copper Ridge, TN is seeking an Activity Assistant for the Memory wing to support the Activities Director in planning and delivering a creative program. The role requires communicating with residents, families and staff and ensuring care plans meet physical, mental and psychosocial needs.

Experience in long-term care and dementia training are preferred. Must be willing to learn and work with residents of varying abilities in a supportive team environment.

Qualifications

• Preferable one-year experience in a long-term care facility.
• Requires dementia training.

Responsibilities

• Participate in planning and conducting of individual, small and large group activities.
• Assist in providing communication between employees, residents, families and others to meet resident needs.
• Participate in community planning related to facility interests and resident needs.
• Assist in development of monthly activity calendar and attendance records.
• Assist in developing action plans to correct quality deficiencies as requested by Activities Director.
• Participate in discharge planning and assessment documentation.
• Assist in arranging transportation for residents for outings, appointments or discharge.
• Develop monthly activity schedules including outings and in-room activities for bed-bound residents.
• Encourage resident participation in self-initiated activities and provide materials including Braille or audio books as needed.
• Assist with assessment documentation and quarterly progress notes.
