Job summary

Wesley Willows in Rockford, IL seeks a Memory Wellness Center Well-Being Supervisor to lead brain health programming for Independent Living and Assisted Living residents. The role blends hands-on facilitation with team leadership, promoting a dignity- and abilities-focused approach to memory care.

You will supervise Well-Being Coordinators, run cognitive coaching sessions, plan clinics, build relationships with residents and families, and collaborate across departments to support resident

Qualifications

• Minimum 2+ years in a Senior Living or Dementia Care setting.
• 2+ years experience working with persons living with dementia.
• Valid driver’s license preferred/required depending on duties.
• Strong training and onboarding experience.
• Excellent written and verbal communication skills.

Responsibilities

• Supervise and coach Well-Being Coordinators and Associates, including scheduling, onboarding, training, and performance management.
• Lead day-to-day MWC operations: group sessions, cognitive coaching, wellness lunches, presentations, and support groups.
• Plan and run annual cognitive assessment clinics and Virtual Dementia Tour trainings with tools like SLUMS and DBAT.
• Build relationships with residents and families, providing cognitive coaching and caregiver support.
• Partner with the Assistant Director to develop, pilot, and scale the Prism Program Portfolio.
• Deliver brain health education and presentations to residents, families, staff, and the community.
• Collaborate across departments to support resident well-being.
• Maintain accurate program documentation, budgets, and quality-improvement tracking.
