Job summary

Wesley Willows in Rockford, IL is seeking a Memory Care Assistant for part-time evenings and weekends. You will implement person-centered engagement programs, document resident care using EMR, and collaborate with the care team to ensure safety and rights are upheld.

Required: high school diploma or GED, 1 year in dementia care or senior living, and EMR experience. CNA licensure in Illinois is preferred; 18+ with a strong caregiving orientation is essential.

Qualifications

• High School Diploma or GED required.
• Minimum of 1 year experience in a Senior Living, Dementia Care, or Rehabilitation setting required.
• Minimum of 1 year experience working with individuals living with dementia required.
• Current and active CNA or related healthcare certification/licensure in Illinois preferred.
• Computer literacy including EMR experience required.

Responsibilities

• Implement and facilitate person-centered resident engagement programs under the Well-Being Coordinator.
• Observe, assess, document, and contribute to resident care and service plans using EMR systems.
• Support resident participation in group and one-on-one engagement opportunities.
• Collaborate with interdisciplinary team including Nursing to ensure continuity of care and resident safety.
• Utilize engagement resources and calendars to deliver programs.
