Job summary

Wesley Willows Corporation in Rockford, IL seeks a Memory Care Assistant to support resident engagement and care in a dementia-friendly environment. This part-time role offers evenings and weekend shifts with a holiday rotation.

You will document resident care in EMR, collaborate with Nursing, and help provide person-centered activities. CNA certification is preferred; you must be 18+ and have at least one year of dementia care experience.

Qualifications

• High School Diploma or GED required.
• Must be 18 years of age or older.
• Minimum of 1 year of experience in a Senior Living, Dementia Care, or Rehabilitation setting.
• Minimum of 1 year of experience working directly with individuals living with dementia.
• Current CNA or related healthcare certification/licensure in the State of Illinois (preferred).
• Computer literacy, including experience with EMR or similar documentation systems.

Responsibilities

• Implement and facilitate person-centered resident engagement programs under the direction of the Well-Being Coordinator.
• Observe, assess, document, and contribute to resident care and service plans using EMR systems.
• Support resident participation in group and one-on-one engagement opportunities by fostering creativity and inclusion.
• Collaborate with Nursing and interdisciplinary team to ensure continuity of care and resident safety.
• Utilize best-practice engagement resources and calendars while following established schedules.
• Demonstrate professionalism, compassion, and flexibility; participate in training and onboarding.
