Job summary

Elwood Health & Rehabilitation seeks a dedicated Memory Care Program Coordinator to lead care planning and supervise staff in our Illinois facility. You will work closely with residents, families, and medical teams to ensure compassionate, person-centered dementia care aligned with CMS guidelines.

The ideal candidate has Alzheimer’s certification or willingness to obtain it, a health care supervisory background, and strong organizational and communication skills to support quality resident

Qualifications

• Alzheimer’s Certificate or willingness to obtain certificate.
• BSW, recreational therapist or equivalent education.
• Working knowledge of Alzheimer’s and Dementia.
• Supervisory experience in healthcare/long-term care preferred.
• Read, write and speak English clearly.
• Thorough knowledge of CMS regulations.

Responsibilities

• Support the community’s philosophy of care and achieve its goals.
• Assign staff to residents and supervise duties to ensure completion.
• Make daily rounds to ensure Care Plans are followed and updated.
• Coordinate rounds with physicians and document visits.
• Ensure well-being of residents and communicate declines to families.
• Maintain infection control and universal precautions.
