Wider Circle is seeking a Case Manager - Member Services Representative to assess member needs, advocate for services, and track progress. You will collaborate with community partners to connect members with resources and support holistic well-being as part of Wider Circle’s Love, Learn, Grow philosophy.

You will empower members by guiding them through service access and ensuring follow-up to address evolving needs within a fast-paced nonprofit setting.

Qualifications

• Experience in case management, social work, or healthcare advocacy.
• Strong communication and interpersonal skills.
• Excellent organizational and time management abilities.
• Ability to collect, analyze, and report data.
• Knowledge of community-based services and social programs.

Responsibilities

• Member Needs Assessment: conduct intake interviews to determine member needs.
• Advocacy: connect members with essential services and resources.
• Progress Tracking: monitor and document progress with follow-ups.
• Data Collection & Reporting: collect, analyze, and summarize results for leadership.
• Collaboration: work with community partners and providers to ensure comprehensive care.
• Member Empowerment: educate members on resources and guided processes.
